Duplicate User in Role Request Blocking Submission


We are experiencing an issue with the user access review. When looking at a user access review, we are seeing the same username listed twice under a request for one role. However, when this happens, the action field for the duplicated user is empty. When this request is submitted, it is erroring out because of the empty action field. However, when this is changed or updated, it just updates the username's field that is not empty. WIth this issue, the empty field will never be not empty (in order to submit the request). Please help.

    What SP level is GRC at? check if following notes are relevant for your system.
