Carry out the following steps to build a cause hierarchy:
Under Node types , specify the levels that your cause hierarchy is to include. If, for example, your hierarchy is to include five levels, create node types 1 through 5. The numbers 1 to 5 here represent the respective hierarchy level: "1" is the highest hierarchy level, "5" is the lowest.
Assign an icon to each level if required. The icon appears in the application in front of the respective node in the hierarchy.
Select Primary Difficulty Source

Unsafe Act
Under Cause hierarchy, specify the key and the description for your cause hierarchy and set the INC/ACC log indicator. The namespaces for the cause hierarchy key are "Y*" and "Z*".(Example ZEHS_IAL_STD)
Under Nodes for hierarchy, specify which hierarchy nodes are to appear in your hierarchy. With the exception of causes in the highest hierarchy level, always assign a predecessor cause.

This is the procedure to build a new cause hierarchy for the incident/accident log.
