Email Template Configuration for CLM, needs assistance

Can anyone help me with editing of the previous template(left) to the proposed one(right).

Any link regarding the same configuration would help, too. Upon saving the changes to Setup > Message Templates, do I need to edit something like script or code?

Please advise.
