If EA-PLM Is not Activated , Is Revision Field will be in Input Mode or Hidden in Maintenance Notification creation (IW21)
being all Config is done Correctly in Spro [ Input Tick ]

Have you checked that field active after doing all the configurations correctly??
It is not showing in start date/ start(040) end Screen area.
