How to add Footer at the end of each Header data in adobe form ?

Hello,

I have a query regarding Adobe form design.I am displaying the form in 3 parts:

1.Header

2.Item data

3.Footer

I am using an Overflow Leader functionality to distinguish the header and Item-level data and it is working fine.The problem is to link the header and footer. I want to display the footer at the end of each Header data as follow:
